Faith-Based Health Learning Community & Leadership Series

We invite you to join a learning community of representatives from various Faith-Based Organizations (FBOs) around the state who are committed to boosting health in their communities by elevating health-focused service. This opportunity is intended for Black / African American faith-based groups across the State of New Mexico.

This learning cohort, led by experts in community health, to empower faith communities to develop or enhance their community and congregation health initiatives addressing health disparities in Black communities. A maximum of two people from each congregation may register as part of the cohort. Each cohort participant is eligible for a stipend of up to $200, based on program participation. Each faith community may be reimbursed for up to $250 to support the completion of their health project.

· Virtual Orientation Session: Monday, March 24, 2025

· In-Person Faith Based Health Summit April 3-4, 2025

o April 3: 6-8 pm Reception at Nexus Blue Smokehouse, 1511 Broadway Blvd., SE, Albuquerque, NM 87102.

o April 4: 9am -4pm Faith-Based Health Summit at New Hope Full Gospel Baptist Church, 1901 Pennsylvania St NE, Albuquerque, NM 87110.

· Post-Summit Virtual Workshops, May and June 2025

Applicants must complete the following:

1. Completion of the application via link (provided below)

a. Upload a letter of support from your organization’s faith leader or board member indicating the institution's commitment to the entire program. The letter of support must be on the organization’s letterhead and include a registered EIN.

b. Applicants should be prepared with short answers (200 words or less) for the following questions

i. Briefly describe the program and list the health topic/s currently supported by your faith-based organization or that your organization may be interested in initiating (e.g., providing emergency/triage services, promoting diabetes and heart disease prevention, promoting vaccines, promoting nutrition and exercise, providing health screenings, walking/movement, etc.)

ii. What does your team (you and your co-participant and your faith leader hope to gain from this Learning Community and Summit experience? Describe your brief vision

2. Past or current health service or health ministry activities are not required to be considered viable applicants for this program. Organizations with all sizes and history of prior experience are welcome to apply

3. Applications are due by March 20, 2025

4. Applicants will be notified by March 21, 2025

Information for awardees

1. If accepted, each participant is eligible for a stipend of up to $200. A W9 and Authorization for Direct Deposit must be completed for each participant. Stipend amount is based on program participation

2. Applicants from congregations from organizations minimum of 75 miles from the Albuquerque in-person event (April 4, 2025), may request to receive compensation of 70 cents/mile (documentation required and uploaded) and one-night of hotel on April 3, 2025.  Applicants are responsible for making their own hotel reservations (maximum of two rooms per organization). 

3. Program reimbursement for the Health Project will be awarded to the congregation following the completion of the Faith-Based Learning Community Program.

Application
